Observations concerning required paperwork submitted for new and recertifying drivers.

Hello to all! I hope you are having a fantastic day!

We will approve drivers as usual so we don’t hold them up for certification, but you must forward COPIES of all paperwork to us within 7 days of the day they are submitted. We will need 7 pages from you, T-9 (2 pages), Pretrip (1 page) and Driving Evaluation (4 pages). Failure to forward completed T-9, pretrip and driving evaluation forms could result in inactive status.

Since implementing the request for COPIES of the T-9, pretrip, and driving evaluation for EACH driver completing the OBI training, we have received close to 200 packets. In reviewing these packets, we have noticed the following rather common mistakes:

1.      T-9 areas of concern

a.   Fill out all information in the header of the form. Use driver's complete name, not just "Chris" or "Nick"

b.   On the back side of the form, list areas covered and times for both new AND recertifying drivers. Use 1, 2, 3, 4, not 1-4. List training time in minutes. Don’t forget to total them up when you’ve completed the training/evaluation. Check off each item as it is covered. All items should be checked for new drivers, * items for recertifying. Don’t forget to write in the pre-trip and driving evaluation scores both on the back and on the front. Tally the number of time driver practiced load, unload, etc. and write this number on the line. Do not stop counting when you hit 10 - we need the actual number there.

2.      Pre-Trip areas of concern

a.   List mileage – I know, I know, you’re not going anywhere, but list the mileage and the time that you start, then list the time you finish. Do the math. If you're testing pretrip and driving evaluation at the same time, you must indicate that by using the mileage and time at the begining of the pretrip then after the driving evaluation is completed and the post trip has been done, enter the time and mileage as the stop info.

b.   Mark out those things that don’t pertain to the bus being checked. Mark N/A on the line.

c.   Check off each item as they name it, then put a check on the line when they’ve named everything on the line. BLANK LINES INDICATE AN ERROR.

d.   Do not count multiple omissions on the same line as more than one error.

e.   Mark score at top when finished, ending time, and total amount of time it took, enter score on T-9 in 2 places.

3.      Driving Evaluation areas of concern

a.   Make sure all information in the header on page 1 is complete. Mark start mileage and time on the correct lines.

b.   Mark mistakes in box, mark number of errors and total points deducted at bottom of page, and check when the page is complete. Do this on each page.

c.   On pages 2-4 be sure to fill in driver's complete name and date at the top.

d.   Don’t forget to mark the number performed both left and right on pages 2 and 3. You must do ONE for each side. If your route has more than one, you must grade only one.

e.   Go back to page 1, enter end mileage and time, do the math.

f.    Enter demerits (number of points to deduct) and score, enter score on T-9 in 2 places.

g.   Sign evaluation, and ask driver to sign.

4.   Last but not least, DESTROY all outdated forms. These new forms are the ONLY forms to be used for certifying or recertifying drivers, and are the same forms used throughout the state. All correct versions were distributed to all OBIs attending an OBI meeting this spring, no matter which region. Those attending the East Region’s meeting would have received these forms in the packet they were given on May 20th, 2010.
