The School Based Telehealth Nurse provides support to the Provider, provides patient and family education, and works cooperatively with school nurses, practice office staff, and the practice manager. This position will require transportation to drive to various school buildings to proctor telehealth visits.
Responsibilities:
- Works collaboratively with the Provider, school district, and the medical office.
- Validates patient appropriateness for a telehealth appointment.
- Validates proper consent is in place for the telehealth appointment.
- Completes full intake using nurse intake form and ensures all items are completed. Obtains vital signs, height, weight, and current accurate list of medications and all pertinent information needed for the provider to assess the patient during the telehealth appointment.
- Sends communication to the medical office requesting a telehealth visit.
- Confirms appointment date and time with the medical office.
- Prepares patients for telehealth visit in a timely fashion, making sure to identify patients.
- Completes telehealth visit by following Provider’s direction.
- Performs POC testing if directed by the Provider.
- Ensures if POC requires a send out, specimens are placed in the lab box for pick up.
- Maintains POC supplies in accordance with guidelines, CLIA policies and procedures.
- Ensures accurate and complete documentation in accordance with policy and protocol.
- Correctly educates patients for all testing and medications that are ordered.
- Completes discharge sheet according to the Prover’s instructions.
- If the telehealth visit is a student, ensures the student gets back to the school office safely.
Minimum Requirements:
- Graduate of an approved Medical Assisting Program or License Practical Nurse Program
- Current certified/registered medical assistant or Licensed Practical Nurse
- Current up-to-date CPR training
- Valid Driver’s License and ability to commute regularly between clinic and school buildings
- Must adhere to office policy of yearly TB testing, OSHA and HIPAA training
- Evidence of good health as evidence of infrequent absenteeism
- Ability to meet and interact with the public in a mature, courteous and professional manner
- Effective interpersonal communication skills
- Ability to work efficiently and accurately under conditions of high stress
- Must be able to work as a team and independently
- Positive, friendly, and compassionate
- Must possess the ability to multi-task and work in a fast-paced environment
Additional Details:
- Monday – Friday, 8 am to 12 pm on schools days
- Additional days may be required as needed for training
