Skill-Building for Transition Program
Skill-Building for Transition is an IEP Service for Belmont County Schools.
A successful transition from school to adulthood for students with disabilities is vastly improved when they can engage in meaningful work and life experiences while still in school. The Transition Specialist helps students build skills through targeted activities listed in the youth’s IEP, including everyday life, social, and employment skills. Skill-Building for Transition also acts as a stepping stone to competitive employment.
Referral Criteria
- Is the student age 14 or older?
- Is there a developmental disability?
- Is there an intellectual disability?
- Is there a medical diagnosis?
- What is the goal or skill development desired?
A district representative should complete the form linked to the right and submit the students IEP, ETR, an Emergency Medical Form, an Health Care Plan (if applicable), and BIP (if applicable).
The Transition Specialist reviews submitted information and will contact the district representative.
